Become a New Patient
Joining our practice is simple and convenient, with same-day and next-day appointments available for new patients.

Complete Your New Patient Packet:
– Email the completed forms to info@orchardmedgroup.com or fax them to 888-927-0461
– You can also bring them to your appointment or pick them up in person at one of our offices
– If you can’t complete the forms ahead of time, please arrive 20 minutes early to complete them in the office.
A few things to remember before your visit:
– Check with your insurance provider to confirm if you need to update your Primary Care Physician (PCP) in their system before your appointment.
– Bring a copy of your driver’s license, your insurance card, and an up-to-date list of your medications (if applicable).
